Matt Stone and Trey Parker first invented it while filming one of their movies, either Cannibal! The Musical, Orgazmo or BASEketball, and it means a “stupid joke”. Mutt is correct in that they first used it in South Park’s Season 3 (1999) episode “The Succubus” for the character Mr. Derp. They’ve reused it a few times since, most notably for the Season 6 (2002) episode “The Biggest Douche in the Universe”
